Ontario Estate Court Forms Updated and Available in Estateably

The Ontario courts updated several key estate court forms following recent amendments to the Rules of Civil Procedure (O. Reg. 72/25), effective August 13, 2025. A full summary of the regulatory changes can be found on the Ontario Regulatory Registry.

The following six forms, now carrying a January 2025 version date, have been revised and are available in Estateably:

• 74A – Application for Certificate of Appointment of Estate Trustee
• 74.1A – Application for a Small Estate Certificate
• 74B – Affidavit of Service
• 74B.1 – Lawyer’s Certificate of Service
• 74.1B – Request to File for (Amended) Small Estate Certificate
• 74J – Application for Certificate or Confirmation of Appointment
